為維護本中心實驗區域之安全衛生、儀器運作與資訊保密,申請人進出本中心實驗區域,須詳閱並承諾遵守以下規範:

安全衛生與法規遵循
- 法規遵守:進出本中心須嚴格遵守實驗區域安全衛生規章、潔淨室規則及國家相關安全衛生法令。
- 安全承諾:本人保證於實驗期間,均確實遵守本中心之安全衛生管理規章、規定、辦法或工作守則;若因未遵守上述規範而發生意外,概由本人承擔一切後果。

儀器操作與物品管理
- 標準操作:任何儀器、設備之架設與使用工具均須符合安全規格及標準。申請人須熟悉安全操作方法、防護具使用、緊急事件應變準備及操作後之復原,並確實配戴必要之防護具。
- 物品管制:實驗區域內所有物品,未經相關主管同意前,嚴禁任意搬動及使用。
- 實驗限制:未經許可,不得從事實驗計畫以外之實驗、不做超越儀器設定常規之操作,亦不得使用額外之化學品及樣品。
- 危害物管理: 若需使用危險或有害物質,應要求供應商於容器上標示符合「危害性化學品標示及通識規則」規定之標籤,並提供物質安全資料表(SDS)交給安全衛生人員。

健康狀況與人員照應
- 健康通報:申請人若有突發性疾病或症狀(如:突然昏倒、暈眩、癲癇、心臟病、血糖過低、自律神經失調等),須事先知會指導教授與實驗區域負責人,經評估後始得申請使用。
- 結伴作業:具前項疾病或症狀之使用者,進入實驗區域進行實驗時,任何時段皆須有兩人(含)以上於同一區域工作,以便互相照應。

門禁磁卡與帳號管理
- 磁卡使用:磁卡嚴禁轉借他人使用,違者依實驗區域規定嚴處。若遺失應儘快通報相關人員取消門禁及機台電腦權限,以免遭冒用,並重新申請新卡。
- 效期確認:為確保會員資料正確性,本中心於每學期開學後進行線上磁卡確認作業,請依公告辦理。
- 退卡與賠償:畢業或離校時需至「製程與量測分析服務系統/會員服務/會員退卡申請」辦理退卡。磁卡押金每張 300元,於完成退卡作業後無息退還;若遺失或損壞,須負責賠償300元。

保密規範與法律責任
- 保密義務:申請人於實驗區域內應嚴格遵守保密規範。未經本中心明確授權,嚴禁拍攝機台設備、內部設施或任何實驗資料。
- 違規處置:若查獲申請人有「未盡保密之責、違規複製與洩漏」或「違規散佈相關資料(如私自拍攝並上傳至社群媒體)」等情事,本中心將即刻要求其將相關影像與資料下架刪除,並立即予以停權懲處(如註銷門禁權限、取消機台預約及使用資格等)。
- 法律責任:承上款,若因違規洩密行為造成本中心或相關授權廠商之營業秘密外流或財產損失,申請人須負擔所有損害賠償責任,本中心並將依法追究相關民、刑事之法律責任。
To safeguard occupational safety and health, ensure the proper operation of instruments, and maintain the confidentiality of information within the laboratory areas of the Taiwan Semiconductor Research Institute (hereinafter referred to as the “Institute”), each applicant seeking access to such areas (hereinafter referred to as the “Applicant”) shall carefully read and undertake to comply with the following requirements:

Occupational Safety and Health and Regulatory Compliance
- Compliance with Laws and Regulations: The Applicant shall strictly comply with the safety and health regulations applicable to the Institute’s laboratory areas, cleanroom rules, and all applicable national occupational safety and health laws and regulations when accessing the Institute.
- Safety Undertaking: The Applicant hereby undertakes to fully comply with the Institute’s safety and health management regulations, requirements, procedures, and work rules throughout all experimental activities. In the event of any accident arising from the Applicant’s failure to comply with the foregoing requirements, the Applicant shall bear full responsibility for all resulting consequences.

Instrument Operation and Management of Items
- Standard Operating Requirements: The installation of any instruments or equipment, and all tools used, shall conform to applicable safety specifications and standards. The Applicant shall be familiar with safe operating procedures, the use of personal protective equipment, emergency preparedness and response procedures, and procedures for restoring instruments, equipment, and work areas to their proper condition after use. The Applicant shall wear all required personal protective equipment.
- Control of Items: No item within the laboratory areas shall be moved or used without the prior approval of the relevant supervisor.
- Restrictions on Experimental Activities: Without prior authorization, the Applicant shall not conduct experiments outside the scope of the experimental plan, operate instruments beyond their prescribed operating parameters, or use additional chemicals or samples.
- Management of Hazardous Substances: Where the use of dangerous or hazardous substances is required, the Applicant shall require the supplier to affix labels to the containers in accordance with the Regulations for the Labeling and Hazard Communication of Hazardous Chemicals and to provide Safety Data Sheets (SDS), which shall be submitted to the occupational safety and health personnel.

Health Conditions and Mutual Assistance
- Disclosure of Health Conditions: An Applicant with any medical condition or symptoms that may give rise to a sudden episode, including but not limited to fainting, dizziness, epilepsy, heart disease, hypoglycemia, or autonomic dysfunction, shall notify their academic advisor and the person responsible for the relevant laboratory area in advance. An application for use may be submitted only after an assessment has been conducted.
- Buddy System: When a user with any condition or symptoms specified in the preceding paragraph enters a laboratory area to conduct experiments, at least two persons, including that user, shall be working in the same area at all times to provide mutual assistance.

Access Card and Account Management
- Use of Access Cards: Lending an access card to another person is strictly prohibited. Any violation shall be subject to strict disciplinary action in accordance with the regulations governing the laboratory areas. If an access card is lost, the Applicant shall promptly notify the relevant personnel to have the associated door access and equipment computer access privileges revoked to prevent unauthorized use, and shall apply for a replacement card.
- Periodic Access Card Verification: To ensure the accuracy of member information, the Institute shall conduct online access card verification after the commencement of each academic semester. The Applicant shall complete the verification in accordance with the applicable announcements.
- Card Return and Compensation: Upon graduation or departure from their educational institution, the Applicant shall apply to return the access card through (Process and Measurement Analysis Service System / Member Services / Access Card Return Application). A deposit of NT$300 per card shall be refunded without interest upon completion of the card return procedure. If the card is lost or damaged, the Applicant shall be liable for compensation in the amount of NT$300.

Confidentiality Requirements and Legal Liability
- Confidentiality Obligations: The Applicant shall strictly comply with all confidentiality requirements within the laboratory areas. Without the Institute’s express authorization, photographing or otherwise recording any machinery, equipment, internal facilities, or experimental data is strictly prohibited.
- Measures in the Event of a Violation: If the Applicant is found to have failed to fulfill their confidentiality obligations, engaged in unauthorized copying or disclosure, or disseminated relevant information without authorization, including taking photographs without permission and uploading them to social media, the Institute shall immediately require the Applicant to take down and delete the relevant images and information. The Institute shall also immediately impose a suspension of privileges, including such measures as revocation of access privileges, cancellation of equipment reservations, and withdrawal of eligibility to use equipment.
- Legal Liability: Where an unauthorized disclosure referred to in the preceding paragraph results in the disclosure of trade secrets or causes property loss to the Institute or the relevant licensing companies, the Applicant shall be liable for all resulting damages. The Institute shall further pursue the Applicant’s civil and criminal liability in accordance with applicable law.
